There are several child who need addtional emergency contacts and permission to pick up (2)(c)(d) There were two children files that did not include the last day of enrollment (2)(g) There was one child who did not have dental provider infomation attached to enrollment form (4)(e) There were two child that did not have complete consent to senk medical care and treatment of the child in the event of an emergency. Form needs to include Licensee's name and child's name on the form (4)(g)
